California OSHA issued 10 citations and $242,600 in penalties to Platinum Pipeline, Inc., after a worker died in a trench collapse. Inspectors concluded that the company failed to train workers on excavation hazards; protect workers from excavated materials; and adequately slope, shore, or shield an excavation wall.
